粤东地区首批16例非亲缘造血干细胞捐献者临床特征与安全性分析

摘要:

目的:分析广东省粤东地区首批非亲缘造血干细胞捐献者的人口学特征、干细胞动员效果及短期安全性。方法:采用回顾性研究方法,纳入2023年11月至2025年9月于汕头大学医学院第一附属医院行造血干细胞捐献的16例健康非亲缘供者。所有供者均接受重组人粒细胞集落刺激因子[10μg/(kg·d),分两次皮下注射]动员,并于第5天开始采集。分析采集前后关键实验室指标、采集物质量及其不良反应。结果:16例供者中位年龄27岁,男性为主(81.3%)。学生是主要群体(37.5%,6/16),其中5人为医学生。动员期间,56.3%(9/16)的供者出现骨痛;另各有1例(6.3%)供者报告阴茎勃起异常及新型冠状病毒感染,均未影响采集。所有供者均采集成功,CD34+细胞中位数为8.1×106/kg(受者体重)。单采后,68.8%(11/16)的供者出现轻度低钾血症,经口服补钾后迅速纠正。结论:本研究首次报告了粤东地区非亲缘造血干细胞捐献的临床数据,证实了在该区域现有体系下动员与采集过程安全、有效。学生捐献者占比高及单采后低钾血症发生率高,是未来优化供者管理的重要方向。

Abstract:

Objective: To analyze the demographic characteristics, mobilization efficacy, and short-term safety of the first cohort of unrelated hematopoietic stem cell donors in Eastern Guangdong. Methods: A retrospective study was conducted on 16 healthy unrelated donors who underwent hematopoietic stem cell donation at the First Affiliated Hospital of Shantou University Medical College between November 2023 and September 2025. All donors received recombinant human granulocyte colony-stimulating factor [10 μg/(kg·d), administered subcutaneously in two divided doses] for mobilization, with collection initiated on day 5. Key laboratory parameters pre-and post-apheresis, quality of collected products, and adverse events were analyzed. Results: The median age of the16 donors was 27 years, with a male predominance(81.3%). Students constituted the largest subgroup(37.5%, 6/16), including five medical students. During mobilization, bone pain was reported in 56.3%(9/16) of donors. One donor(6.3%) experienced priapism, and another(6.3%) tested positive for SARS-CoV-2; neither event interrupted the collection process. All collections were successful, with a median CD34~+cell count of 8.1×106/kg(based on recipient weight). Post-apheresis, mild hypokalemia occurred in 68.8%(11/16) of donors, which was promptly corrected with oral potassium supplementation. Conclusion: This study provides the first clinical data on unrelated hematopoietic stem cell donation in Eastern Guangdong, demonstrating that mobilization and collection within the regional system are safe and effective. The high proportion of student donors and the frequent occurrence of post-apheresis hypokalemia highlight key areas for optimizing donor management strategies.
